Position Announcement:  DRS/Medical Case Manager

 

Position:

 

South Side Help Center is seeking a to fill a vacancy of a 50% DRS Home Service Program; 50% Medical Case Manger.  Duties include, but are not limited to assessments, service planning, follow up, home visits, linkages and community services and programs, information disseminating, resource management, advocacy, and discharge planning.

 

Principal duties and Responsibilities:

 

·         Complete comprehensive psychosocial assessments, identitify needs, provide referrals, and assist clients with application process.

·         Conduct in home assessment with DRS customers to meet customer need for assistance with activities of daily living eligibility for program  services.  Conduct in office assessment for Medical customers.

·         Provide monthly home visit for DRS customers to ensure customer health and safety, supportive services needs are met.

·         Provide education, information and linkages to programs.

·         Assess customer eligibility for financial assistance programs and assist customer with applications as appropriate.

·         Review/revise service plans and perform follow-up to determine the quality of services provided to customer.

·         Provide and support customer in the follow up with medical care, medication adherence, risk reduction, nutrition, mental health and substance use.

·         Locate, identify and utilize community services and program.

·         Develop and maintain required records, report and continuing education requirements.

 

Qualifications:

 

Case management experience highly preferred.  Knowledge and/or experience in working  with various populations, LGBT, and HIV+/PWA required.  Driver’s Licence, insurance, good driving record  and access to a reliable transportation.  Strong written and verbal communication skills required. 

 

Mandatory Job Qualification

           

Minumum of Bachelors’ degree in social work field
