The Pediatric AIDS Chicago Prevention Initiative (PACPI) is a privately funded collaborative enterprise that brings together public and private institutions and community-based organizations into a coordinated continuum of primary and secondary prevention efforts to achieve our goal – the elimination of pediatric AIDS in Chicago.
The PACPI Perinatal Enhanced Case Management program targets the hardest-to-reach, hardest-to-link women who need extra assistance to successfully engage in the complex combination of care and services needed to ensure a healthy outcome for both mothers and babies. Women are referred into the program at any point in their pregnancy or shortly after delivery of their child. Referral sources include the Ryan White Case Management system at the AIDS Foundation of Chicago (AFC), the 24/7 Perinatal HIV Hotline, and direct referrals from community-based agencies/HIV care programs. Cases are assigned to case managers based on current caseload, specific needs of the client and specific strengths of the case manager.

Position Description:
FACILITATES LINKAGES WITH: Primary care/prenatal care/well-child care; substance abuse treatment; housing, food services, transportation, mental health, Department of Human Services, Department of Child and Family Services; other services identified as needed. This position will concentrate on the Chicago metropolitan area and Cook County. Office space in Chicago will be provided.
RESPONSIBILITIES:
• Engage HIV-infected pregnant women in intensive case management throughout pregnancy and six months post-partum to promote adherence to prenatal care visits, substance abuse treatment, perinatal prophylaxis, and well child care.
• Facilitate active linkages with services, scheduling appointments and arranging/providing transportation to services.
• Participate in on-call rotation for 24/7 hotline, provide emergency enhanced case management to HIV+ pregnant women.
• Provide emergency case management services for women identified as preliminarily HIV-positive through the rapid testing program at labor and delivery hospitals in the Chicagoland area (including home visits) and follow through linkage to care appropriate to the result of confirmatory testing.
• Conduct initial assessments of basic needs, psychosocial needs, medical needs and identify linkages and referrals needed.
• Maintain daily contact with client and prenatal care providers and support adherence to care.
• Work with HIV+ women to develop a client-centered and concretely defined plan for success, ongoing reproductive health, HIV prevention and transition to less intensive case management services at six months post partum.
• Facilitate the development of a birth plan with medical and other social service providers
• Conduct individual and family centered HIV prevention counseling.
• Participate in regular multi-disciplinary staffing meetings to present and assess client needs and progress.
• Establish, develop and maintain relationships with key service providers.
• Ensure client linkage to perinatal network services and/or IDHS Family Case Management via co-located services.
• Work within the AFC coordinated case management system.
Desired Qualifications:
SKILLS:
• Client-centered counseling
• Good understanding and background in maternal child health, substance abuse care and treatment as well as HIV care systems
• HIV transmission, prevention and treatment
• Setting boundaries and goals
• Basic computer skills (MS Word and MS Excel)
• Bilingual/bicultural Spanish language is strongly preferred but not required
TRAINING:
• Safety in home visiting
• Recognition and addressing psychotic episodes and response to psychosocial crises
• Orientation and regular training through the Northeastern Illinois Case Management Cooperative
• AFC Medical Case Management certified, or certification must be achieved through Case Management Competencies training
• Recognizing and addressing issues of family violence
• Client-centered counseling
• Perinatal HIV prevention (antepartum, intrapartum and postpartum)
• Additional topics as requested or as needed by the caseload
